Description:

This is a U.S. Dept. of Agriculture funded nutrition supplemental food program for pregnant, postpartum (to 6 months) and lactating women and children up to age 5. This program enables eligible clients to receive special checks/vouchers for the purpose of purchasing nutritionally qualified products. Individual nutritional counseling is provided.

Hours:

Mon - Fri, 9 am - 5 pm; Tues, 8 am - 6 pm;

Eligibility Requirements:

Pregnant, post partum (to 6 months), lactating women and children up to age 5 from families with lower incomes are eligible.

Admission Requirements/Documents:

Proof of income, physician's referral form (physician must document nutritional risk factors), birth/baptismal certificate, proof of residence, Medicaid card; proof of income within eligibility guideline.

Fees/Methods of Payment:Free.
