Description:The Developmental Disabilities Day Treatment Program offers a unique day treatment program for blind and visually impaired individuals with developmental disabilities. The goals of the program are to improve the quality of individuals' lives, assist clients to remain in their communities by making the most of their abilities.
Hours:Mon - Fri, 8:00 am - 5:00 pm.
Eligibility Requirements:Visually impaired; aged 21 and over; emotional and behavorial problems; diagnosis of mental illness. Staff members conduct complete assessment and evaluation to determine eligibility.
Admission Requirements/Documents:Referral by medical and mental health professionals, schools, hospitals, day programs, families or companions; self-referral.
Fees/Methods of Payment:Medicaid.
