Description:This program offers therapy for those recovering from falls, cardiac conditions, amputations, and other disabling conditions. Other services include restorative physical, speech, and occupational therapy for adults.

Hours:Mon - Fri, 8:30 a.m. - 4:30 p.m.
Eligibility Requirements:Adult eligible for restorative therapy.
Admission Requirements/Documents:Call for appointment. / Prescription for therapy (can obtain here).
Fees/Methods of Payment:Medicare assignment; Medicare co-pay; Medicaid, most other insurances.
Access for People with Disabilities:Wheel chair accesssible; extra wide doors; doors that open both directions (in and out); arrangement of furniture, displays & equipment for handicapped; special restroom facilities; grab bars in restrooms. Completely accessible for the disabled.
