Description:The Adult Services Program includes a Day Treatment and Day Habilitation Program, serving more than 150 people with significant and/or multiple disabilities. QCP believes that all people can learn, regardless of the severity of their disabilities. The goal of the Day Program is to help individuals acquire the skills they need to live more independently in the community. Additional services provided are work training and clinical services, which provide social work, rehabilitation counseling, psychological and nursing care.

Hours:Mon - Fri, 8:30 .am - 4:30 pm. Additional Facility: Mon - Fri, 8:30 am - 4:30 pm.
Eligibility Requirements:Developmentally disabled, Queens resident, Medicaid eligible.
Admission Requirements/Documents:Proof of or Medicaid eligibility. / Medicaid-citizenship status.
Fees/Methods of Payment:Medicare assignment; Medicaid; sliding-scale fees; all government programs.
Access for People with Disabilities:Wheel chair at entrance; multiple floors; extra wide doors; doors that open both directions (in and out); arrangement of furniture, displays & equipment for handicapped; special restroom facilities; grab bars in restrooms; flashing emergency lights; sign language.
