Description:House Calls Program is a non-emergency medical care program for frail, homebound elderly. The House Calls physician will visit the elderly and conduct a comprehensive assessment of the special needs. Between visits, a nurse case manager will coordinate support services and coordinate laboratory work-ups, nursing visits, physical therapy, and other health care and social services the patient might need.
