Description:Provides case management, prevention and outreach services to HIV infected and affected individuals and families. Services include: case managers who network with other agencies to obtain entitlements, home and hospital visits, counseling for patients and their families, medical referrals, health fairs, and prevention information.

Hours:Mon, Fri, 9 am - 5 pm, Tue, Wed, Thurs, 9 am - 8 pm.
Eligibility Requirements:All ages.

Access for People with Disabilities:Wheel chair ramp at entrance, wheel chair ramp(s) internally, elevator, extra wide doors, grab bars in restrooms.
