Description:This team services individuals in Eastern Queens who are at risk or are experiencing a psychiatric or emotional crisis which requires mental health intervention, treatment and or support. They provide home visits, assessment of needs, psychiatric assessment and evaluation, supportive counseling, linkage to services, and short-term follow-up.
