Description:
The Momentum Project sustains health and fosters wellness by providing communal meals and supportive services to any person living with HIV/AIDS, their family members, and loved ones in New York City. Momentum creates a caring environment in which hope can develop, the spirit is nurtured, and clients improve the quality of their lives.
Hours:
Mon - Fri, 9 am - 6 pm.
Eligibility Requirements:
HIV/AIDS patients. Has to show the proof of their HIV status.
Admission Requirements/Documents:
Call Momentum's Referral Staff at (212) 691-8100, Ext. 326, between 11:00 am and 3:00 pm, Monday through Friday. / New clients require a letter of introduction from the referral agency, and a copy of their current M11Q form or other proof of HIV status at the intake interview.
