Description:

We provide an integrated long-term program of health care and supportive services on an outpatient basis to persons who would otherwise deteriorate mentally and physically at home.  Services include: nursing, physical, speech and occupational therapy, recreational, dietary and social work services.  Transportation and meals are provided.

Monday-Saturday: 9:00am-3:00pm

Eligibility Requirements:

Adults suffering from Alzheimer's disease or a chronic physical disability.
