Description:The Community Life Skills Program is dedicated to the treatment of the mentally ill as well as the mentally ill chemical abuser. Services offered include: group therapy, individual therapy/counseling, case management services, work skills training, psychiatric services, and chemical dependency counseling.

Hours:Monday-Friday: 9:00am-5:00pm
Eligibility Requirements: Age 18 and older with primary psychiatric diagnosis; can have secondary substance abuse problem.
Admission Requirements/Documents: Mental status, medical, medication and psychosocial history.
Fees/Methods of Payment: Medicaid
