Description:Adult Day Health Care provides nursing and medical care in a social atmosphere to community people with the intent of keeping them in the community and out of a nursing home. Services include a full range of medical clinics, rehabilitation services, full nursing services, and recreational activities.

Hours:Mon -Fri, AM session 7:30 am -3:30 pm, PM session 11 am - 4 pm; Weekends 7:30 am - 1:30 pm.
Eligibility Requirements:Aged 50 and older frail elderly with medical or functional disabilities and/or social and emotional problems. Clients who are wheelchair dependent must be a 1 person assist or able to transfer to the toilet independently.
Admission Requirements/Documents:Complete history and physical from medical doctor, including routine diagnostic testing and tuberculin testing.
Fees/Methods of Payment:Medicaid, private pay ($42.00 daily rate) accepted.
