Description:

This organization provides home health care services to the Medicaid-eligible population. Assists clients with feeding, dressing, personal hygiene, and tasks such as meal preparation, cleaning, and laundry. Services based on in-home assessment by HRA social worker.

Hours:

Mon - Thurs, 9 - 5; Fri, 9 - 3 (Apr-Oct), 9 - 2 (Oct-Apr) (Friday hours change because of Sabbath).

Eligibility Requirements:

Medicaid eligible. Physically or mentally incapacitated and in receipt of Medicaid.

Admission Requirements/Documents:

Medicaid card, M11Q from physician and authorization from HRA home care services program.

Fees/Methods of Payment:Medicaid.
