Description:GuildCare provides adult day health services for visually impaired or blind adults who can benefit from the services of a comprehensive health and social program while remaining in their community. GuildCare seeks to prevent or delay institutional care and provide relief to caregivers by offering nursing supervision; social work services; recreation; nutrition counseling; health education; physical, occupational, speech and vision rehabilitation therapies; ancillary health services; hot lunch; transportation; recreational and skills classes; and field trips. There is also a managed long-term care program, Guildnet.
Hours:Mon - Fri, 9:00 a.m.-5:00 p.m.
Eligibility Requirements:Diagnosed medical problem; visual impairment affecting physical, emotional or functional ability; age 18 and over; self-feed and toilet with assistance; ability to use scheduled door-to-door transportation; physician in the community; complete physical exam upon admission and yearly thereafter.
Fees/Methods of Payment:Medicaid.
