Description:Phase I-III Programs are supervised, intensive supportive, and supportive housing programs for persons at least 18 years of age who have had some psychiatric problems that have prevented them from living independently. Supervised means that you need 24-hour ongoing supervision and monitoring. Intensive supportive means that you will get at least one daily face-to-face visit from staff. Supportive means you will get at a minimum one to three face-to-face visits weekly from staff. Residents work on developing independent living skills. Phase 1 is a dormitory setting. Phase 2A is an apartment setting with counseling seven days/24hours. The third program is called Apartment Program which provides scattered apartments through Queens.
Hours:24 hours/7 days.
Eligibility Requirements:Persons aged 18 and older with a primary psychiatric diagnosis are eligible.
Admission Requirements/Documents:Approved HRA 1990 and TSI referral form.
Fees/Methods of Payment:Clients are their own payees with approved HRA 1990. Their monthly SSI or DSS will cover TSI fees in full.
