Description:

Services include two 5-hour-a-day programs of individualized medical and nursing care; social services; rehabilitative therapies; recreation; nutrition, pharmacy and dentistry services; transportation; and hot kosher lunch. Sessions meet in the morning from 9:30 a.m. to 2:30 p.m. or in the afternoon from 3:00 p.m. to 8:00 p.m.

Eligibility Requirements:

Residents of Queens and Nassau, with medical problems are eligible.

Admission Requirements/Documents:

All participants must have a medical evaluation before being accepted into the program. That service can be provided for applicants without private medical care.

Fees/Methods of Payment:Medicare, Medicaid and private insurance are accepted for these services.
Access for People with Disabilities:Wheel chair ramp at entrance; single floor; extra wide doors; doors that open both directions (in & out); arrangement of furniture, displays & equipment for handicapped; special restroom facilities; grab bars in restrooms; lowered telephones.
