Description:
Provide social services to those in need to improve their quality of life regardless of race, creed, age, religion, color, disability, national origin, sex or marital status.
Hours:
Mon - Thurs, 9 am - 4 pm ; Fri, 9 am - 1 pm; Lifeline information available Mon - Wed, 9 a.m.-4 p.m.. At other times, leave message.
Eligibility Requirements:
Persons aged 60 and over who are frail and/or have a chronic medical condition are eligible. Priority is given to frail seniors with low income and over age 75.
Admission Requirements/Documents:
Proof of income and resources required. Proof of rent (or shelter expenses) and non-reimbursed medical expenses also required for fee adjustment.
Fees/Methods of Payment:Sliding-scale fees; private pay accepted.
Access for People with Disabilities:Office is wheelchair accessible, bathrooms are not.
