Description:Social work services, including assessment, comprehensive care planning, monitoring, and reassessment are provided on a fee for service basis to the high risk frail elderly living in the community. A social worker/case manager trains and supervises a core of volunteers to assist in the provision of services. Referral services are provided to clients of The Partnership Group.

Hours:Mon - Fri, 9-5.
Eligibility Requirements:Senior citizens, after 60 years old.
Admission Requirements/Documents:Call Program.
Fees/Methods of Payment:Private pay, fee can be reduced based on need.
