Eligibility Requirements:Persons requiring intensive nursing services for treatment and rehabilitation but who are not in an acute phase of illness are eligible.
Admission Requirements/Documents:SS card/number, insurance ID, Medicaid card, Medicare card, application form, PRI screen.
Fees/Methods of Payment:$315 per day. Medicaid, Medicare, private health insurance accepted.
