Eligibility Requirements:

Children aged 14 and younger of NYC residents who usually live or work in the area are eligible. Families must meet HRA/ACD financial eligibility requirements. Intake at HRA/ACD Resource Area 08. Call (718) 523-6826

Admission Requirements/Documents:

Birth/baptismal certificate, Social Security card/number, parent's proof of eligibility, and immunization record. Pay stubs.
