This is a voluntary facility with 220 beds providing long-term skilled nursing. There is an adult day care center. Has physical therapy, occupational therapy and speech therapy.

24 hours/7 days

Eligibility Requirements:

Age 65 and over are eligible.

Admission Requirements/Documents:

PRI (Patient Review Instrument) evaluation done by visiting nurse services must be completed before any other services can be given. SS card/number, insurance ID, Medicaid card, Medicare card, proof of lawful admission for aliens, and bank statement are also required.

Fees/Methods of Payment:Fixed fee. Medicaid, Medicare, private health insurance, and private pay accepted.
Access for People with Disabilities:Wheelchair access, braille signs.
