Description:This is a designated case management agency for services to older adults living in Forest Hills/Rego Park. Services include: For the homebound - care plan developed in your own home, home delivered meals, transportation (limited availability), home care (for those on limited income), telephone reassurance, and intergenerational chore. For the frail - Day Center providing socialization and recreation. For the family - respite for Alzheimers caregivers, Alzheimers support group, and consultation on services and entitlements. For the active elder person - Senior Center at Lost Battalion Hall and at the Forest Hills Community House, daily noon meal, recreational and educational programs, volunteer opportunities, and leadership training. | Also available are referrals to other agencies for services not directly provided. "LIFT" program offers socialization for physically and emotionally challenged, mentally sound seniors. Transporta provided. A group also meets twice per week for mentally frail elders who face memory deficits.

Hours:Monday-Friday: 9:00am-5:00pm; Alternate Agency Hours: Monday-Friday: 9:00am-4:00pm
Eligibility Requirements:Older adults living in Forest Hills/Rego Park are eligible.
