Description:

Parker Institute’s Kessel Long Term Home Health Care Program helps older adults receive medical care at home while remaining independent. This program brings individualized medical and nursing services directly to people who are homebound because of physical limitations. Professional staff members visit patients regularly to assess, develop treatment plans and provide ongoing care.

Mon - Fri, 9 am - 5 pm.

Eligibility Requirements:

Frail elderly residents of Queens or Nassau counties needing professional and aide home care services are eligible.

Fees/Methods of Payment:Sliding scale fees. Medicare, Medicaid, private insurance, and private pay accepted.
Access for People with Disabilities:Wheelchair accessible.
