Description:This program offers eligible persons case management, meals on wheels, transportation, shopping/chores, personal care, short-term counseling, information and referral, and assistance with entitlements (social security, medicare, medicaid, food stamps, SSI, real property tax reduction, section 8 housing, SCRIE, and HEAP).

Hours:Mon.-Fri., 8:30 a.m.-4:30 p.m.
Eligibility Requirements:60+ homebound or frail persons in community boards 8, 11, N13, Queens, New York, N.Y., are eligible.
