Hours:

Mon - Fri,  9 am - 5 pm.

Eligibility Requirements:

Persons requiring short-term rehabilitation, skilled nursing, or acute medical care are eligible.

Admission Requirements/Documents:

PRI (Patient Review Instrument) and financial documentation.

Fees/Methods of Payment:Medicare assignment, Medicare co-pay, Medicaid, private insurance, and private pay accepted.
Access for People with Disabilities:Fully accessible.
