Description:

Queens Community House’s Senior Case Management program provides assistance with the process of planning, arranging, and coordinating services and resources for the elderly. We also provide information and referral to services to homebound elderly throughout Queens. Services include: Information and referral, home-delivered meals and assistance with individual benefits and entitlements. Call for a complete list of services.
